Concierge Nurse Advocacy
TriCord Advocacy provides dedicated registered nurse advocates who accompany senior patients to medical visits β ensuring nothing is missed, every question is answered, and every family member stays informed.
A licensed nurse attends appointments, asks the right questions, and ensures your loved one's voice is heard.
Detailed updates sent directly to designated family members after each appointment.
All changes to medications and treatment plans are documented and communicated promptly.
What We Do
We bridge the gap between medical providers and the families who depend on clear, reliable communication about their loved one's health.
A qualified nurse attends office visits with your loved one. We take notes, ask clarifying questions, and make sure the appointment is productive and clearly understood.
After every visit, designated family members receive a clear summary β including diagnosis updates, medication changes, new referrals, and next steps in the care plan.
We track all prescription changes and additions, alert family to new medications, and help ensure seniors understand dosing instructions and potential side effects.
We document treatment plan changes and help families understand next steps β including specialist referrals, diagnostics, follow-up schedules, and lifestyle recommendations.
Our nurses explain diagnoses and instructions in plain language, ensuring your loved one leaves every appointment feeling informed, not overwhelmed.
For unexpected medical situations, our team can be available to assist with urgent provider communication, hospital navigation, and family notifications.
Process
From enrollment to every appointment, we make advocacy simple for both patients and families.
We meet with you and your family to understand your loved one's health needs, current providers, and communication preferences.
Choose from our flexible tiers β hourly, monthly, or Γ la carte services β tailored to your frequency and budget needs.
We match your loved one with a dedicated nurse advocate who reviews their medical history and provider relationships.
Your nurse attends appointments, communicates with family, and keeps a continuous record of your loved one's evolving care.
When a loved one is elderly and living at a distance, medical appointments can be stressful for everyone. TriCord ensures someone knowledgeable is always in the room β asking the right questions and relaying every detail back to you.
